Electronic Health Records and Care Management
Physicians make considerable investments in an EHR, both financially and with staff resources. Care management can be greatly enhanced with a focus on the features and benefits of the EHR.
TMF Health Quality Institute works with physicians concerning EHR and HIT. Our current project as the Quality Improvement Organization for Texas under contract with CMS is focused on assisting physicians with using their EHR to improve care management.
TMF is providing free consulting services to participating physician practices as part of a three-year quality improvement project. Our experienced consultants are working with practices to assess care management workflow, optimize alerts and reminders for disease management and prevention, and monitor selected performance measures for disease management and preventive services.
As the Medicare Quality Improvement Organization for Texas, TMF has extensive experience in health information technology as well as physician office quality improvement projects and is providing these consulting services free to the participating practices.
TMF is working with physician practices to:
- Identify effective, efficient strategies to improve performance with an EHR
- Implement advanced care management processes and patient self-management
- Document care to facilitate successful reporting for PQRI and health plan pay-for-performance programs
- Implement the latest in health care quality improvement methodology
Through this three-year project, TMF’s goal is to impact the rates of two cancer screenings and two immunizations among Texas’ Medicare beneficiaries:
- Mammography
- Colorectal cancer screening
- Influenza immunization
- Pneumococcal immunization
